In compliance with the GLBA, we are providing you with this document, which notifies you of the privacy policies and practices of the Frank Gross Insurance Agency, Inc.

The GLBA further requires that we inform you that you have a right to prevent us from sharing nonpublic personal information about you with a non-affiliated third party for any purpose that is not specifically authorized by law. Your right to prevent us from sharing nonpublic personal information about you with a non-affiliated third party for a purpose that is not specifically authorized by law is called your right to “opt out” of such information sharing.

OUR PRIVACY POLICIES AND PRACTICES

1. Information that we collect:
We collect nonpublic personal information about you from the following sources:

1. Information we receive from you on applications or other forms.
2. Information about your transactions with our affiliates, others or us.
3. Information we receive from a consumer reporting agency,
4. Information we receive from a medical information bureau.

We may disclose all of the information we collect, as described above, about our customers or former customers, to companies that perform marketing services on our behalf or to other financial institutions with whom we have joint marketing agreements. We may also disclose information about our customers or former customers as permitted by law.

2. Other insurance agencies or companies that perform services for us in connection with carrying out an insurance transaction involving you or your policy.
3. Financial institutions with whom we have joint marketing agreements, such as insurance providers.
4. Other types of financial institutions such as premiums finance companies.

B. Notification of Your Rights To Opt Out of Certain Disclosures
As we have indicated in the privacy Policy Notice, we collect certain nonpublic personalinformation about you, and we may disclose that information to certain non-affiliated third parties for purposes other than those expressly permitted by the Gramm-Leach-Bliley Act and the federal and state regulations implementing that Act. If you prefer that we do not disclose nonpublic personal information about you to non-affiliated third parties, you may opt out of those disclosures, that is, you may direct us not to make those disclosures (other than those disclosures that are expressly permitted by the Gramm-Leach-Bliley Act and itsimplementing regulations).

4. Our practices regarding information confidentially and security:
We restrict access to nonpublic personal information about you to those employees who need to know that information in order to provide products or services to you. We maintain physical, electronic and organizational safeguards to protect information about you.

5. Affiliates with whom we share certain information protected by the Fair Credit Reporting Act, unless you tell us not to:
Unless you tell us not to, we may share with our affiliated companies information about your including:

Information we obtain from your insurance application, such as your income or marital status;
Information we obtain from a consumer report, such as your credit score or credit history;
Information we obtain to verify representations made by you, such as your open lines of credit,Information we obtain from a person regarding its employment, credit, or other relationship with you, such as your employment history.

Our affiliated companies who may receive this information are financial services providers, such as: insurance companies, mortgage lenders, and brokers.

If you prefer that we not share this information with our affiliated companies, you may direct us not to share this information by calling us at 859-781-0434 or 800-875-0812 to obtain the Opt Out Form to sign and return to us.

As you know, one of the advantages of placing your business with our agency is that we compare the prices if the many companies we represent to make sure we are offering you the best coverage for the least cost. We are reminding you of that advantage just in case you are considering signing the Opt Out Form referred to in out Privacy Policy Notice. Please be aware that if you sign the form, you will be prohibiting us from obtaining prices from any carrier except the one currently writing your insurance. In our opinion, that could very well work out to your disadvantage. Be advised that we do not- and never will sell any information we have learned about you to anyone. We only use the information we have about you to obtain the best coverage we can for the lowest price and we would not be able to do that if you sign the Opt Out Form.
